Make a Referral to ABC

Required Documents:

1) ABC Program Admission Form. Form needs to be fully completed and signed.

2) Documentation of Disability. Signed medical documentation stating disability or an IEP Evaluation

3) Support Plan (Waiver Funded)

4) Guardianship paperwork (if applicable)

ABC must receive all of the following documentation to accept a referral:

Community-based volunteer skill-building program in Rochester, MN.

Day Support Services

Recreation and leisure program focused on community relationships, involvement, and inclusion.

Employment Exploration

Waivered service that helps a person gain a better understanding of competitive, integrated employment opportunities in their community. Exploration activities and experiences strengthen a person’s knowledge, interests and preferences so they can make informed decisions about competitive employment.

Employment Support Services (Group)

Community-Based work that is served at a 1:2-1:6 ratio with staff supervision. Individuals are paid by ABC. Employment opportunities vary by location.

Employment Support Services (Competitive Follow-Along 1:1)

Service for individuals that are competitively employed (paid by the employer). ABC provides 1:1 check ins to help the individual maintain employment.

Individualized Home Supports

Supporting individuals who live independently or in a family home with building and/or maintaining independent living skills and assisting with community inclusion activities.

Pre-Vocational

Offers job training and support in a fully supervised and supported setting to allow individuals to build on soft skills, helping prepare them for community-based employment.

Waiting List

Most ABC Services have a waiting list. We are unable to estimate wait times.

Ratios

Staffing ratios a crucial in ABC determining the best fit for the person. Please estimate the appropriate staffing ratio for the person.

1:1- Person requires a staff at all times to assist with behavior support plans, personal cares, medication, or other needs. IHS, Employment Exploration, and competitive follow-along are always served at a 1:1 ratio, regardless of support level.

1:4- Staffing ratio allows for assistance with personal cares, mobility needs, medications, hand over hand instruction, and behavioral support needs.

1:6- Person is fairly independent but requires supervision and staffing support. Must be able to have some unsupervised time.

1:10- Person is mostly independent but requires supervision. Person does not need assistance with personal cares, mobility, and medications. Must be able to have some unsupervised time.
