Based School Employment - "School to Work"
The Community
Based School Employment Program serves students with disabilities
or employment barriers.
Goal
To
develop and provide employment and training opportunities
to secondary students with disabilities.
Admission
Criteria
1. Currently enrolled
in a school program
2. Referred by Special Education staff or a Work Experience Coordinator, with
an existing IEP in place.
3. Must have adequate vocational information and medical/pschometric and testing
information as well.
4. Has demonstrated or expressed interest in participating in the program.
5. Vocational needs not met by current system.
6. Have potential to benefit from work experience.
7. Needs support services to maintain community employment.

i.e. On-The-Job- Training, Performance Reviews, Sub-Minimum

Positive
- Upon recommendation
of Transitional Planning Team, referral to appropriate
program

follow through with program plan

fails to maintain contact with case manager

no longer desires services

no longer meets admissions criteria
